A pill for Nexium, when you can get over the counter Prilosec for 67 cents a pill is questionable. By switching patients from Nexium to one of four cheaper medications for heartburn and ulcers, the Pentagon can save many tens of million dollars a year. For decades, private employers, insurers and some State Medicaid programs have refused to pay for some pricey medications by creating a drug list or formulary. The DOD up to now has provided unlimited access to any licensed medication, but now is using its formulary to curtail costs. As of July 17, 2005 four other heartburn drugs will be offered. Zegerid, Protonix, Aciphex and Prevacid, for .00 co-pay per prescription. Patients, who find that the other drugs fail to bring relief and only Nexium works, can, with their doctors backing, request an exemption, or they can.

Metanephric organ culture has been used to determine whether embryonic kidney tubules can be stimulated by cAMP to form cysts. Under basal culture conditions, wild-type kidneys from embryonic day 13.5 to 15.5 mice grow in size and continue ureteric bud branching and tubule formation over a 4- to 5-d period. Treatment of these kidneys with 8-Br-cAMP or the cAMP agonist forskolin induced the formation of dilated tubules within 1 h, which enlarged over several days and resulted in dramatically expanded cyst-like structures of proximal tubule and collecting duct origin. Tubule dilation was reversible upon withdrawal of 8-Br-cAMP and was inhibited by the cAMP-dependent protein kinase inhibitor H89 and the cystic fibrosis transmembrane conductance regulator CFTR ; inhibitor CFTRinh172. For further testing of the role of CFTR, metanephric cultures were prepared from mice with a targeted mutation of the Cftr gene. In contrast to kidneys from wild-type mice, those from Cftr mice showed no evidence of tubular dilation in response to 8-Br-cAMP, indicating that CFTR Cl channels are functional in embryonic kidneys and are required for cAMP-driven tubule expansion. A requirement for transepithelial Cl transport was demonstrated by inhibiting the basolateral Na , K , 2Cl co-transporter with bumetanide, which effectively blocked all cAMP-stimulated tubular dilation. For determination of whether cystic dilation occurs to a greater extent in PKD kidneys in response to cAMP, Pkd1m1Bei embryonic kidneys were treated with 8-Br-cAMP and were found to form rapidly CFTR- and Na , K , 2Cl co-transporter dependent cysts that were three- to six-fold larger than those of wild-type kidneys. These results suggest that cAMP can stimulate fluid secretion early in renal tubule development during the time when renal cysts first appear in PKD kidneys and that PKD-deficient renal tubules are predisposed to abnormally increased cyst expansion in response to elevated levels of cAMP. J Soc Nephrol 17: 3424 3437, doi: 10.1681 ASN.2006030295. DUNCAN J. IRSCHICK tural environment on performance, and emphasizes that studying the effects of habitat on performance is best accomplished in nature, where technically feasible. Second, more studies are needed that draw a direct mechanistic link between habitat use, performance and morphology. For example, few studies have directly tested either the habitat constraint or habitat breadth models, in part because the functional relationship between the habitat and performance is poorly understood for most animal groups. However, these models offer a testable and rigorous method for testing whether species have specialized to different environments. Indeed, where mechanistic approaches have been applied, substantial progress has been made in understanding why species use different habitats, such as in Lepomis sunfish and Anolis lizards. However, the necessary requirements for establishing such a mechanistic link is for biologists to have both a substantial laboratory and field component to their research, which currently is not a top priority for most functional morphologists. The SAA tells me that there are some 400 + people in the U.S. who are paying dues as retired persons, plus another 50 or so from other countries. That's a lot of productive people out there who no longer have to spend most of their waking hours teaching, preparing for teaching, advising students, going to committee meetings, meeting deadlines for contract reports, writing memos to their bureaucratic superior, or otherwise being kept from the pure joy of doing archaeology. Now they are FREE, most of them, to do what they really want to do. And what might that be? Finishing long overdue reports? Writing an account of their career? Setting down their thoughts on the changes in archaeology during their careers? At the suggestion of one of those retired archaeologists--hard at work on analysis and writing up archaeological projects--I have agreed to "edit" a periodic column three times a year perhaps ; in which a retired archaeologist can tell her his colleagues that retirement often provides the opportunity for continued contributions to the field. I seem to know enough of those on the SAA list to last a good many years of three-ayear accounts of neat research being done or even--as with Gordon Willey and now Jim Hester--of novels being written. Anyone who would like to volunteer to describe the lifestyle of a retired archaeologist can contact me personally by email at hadavis uark or "snail" mail at Arkansas Archeological Survey, 2475 N. Hatch Ave., Fayetteville, AR 72704. I retired in 1999, but the Survey graciously gave me an office and a computer so that I should be able to finish undone projects and reports--like many of us retired folk. ; Frederick de Laguna's contribution below is to set an example, not only of this column but of a continuing productive career. Would that we all could sustain this kind of interest and excitement into our 90s! Freddie's archaeological career in the far North is well known, and once you see how she is using her time now, you should read Voyage to Greenland: A Personal Initiation into Anthropology, the first thing she wrote after her retirement 1977, Norton and Co., New York. Religion is practical. Religion must become part and parcel of your daily life. Mere curiosity and a little bubbling juvenile enthusiasm and emotion cannot help you much in your spiritual growth and evolution. Struggle and sustained efforts are needed. Religion is the science of life. It teaches us to live in God and attain immortality. It helps us to enter the realms of eternal bliss and unceasing peace. It removes the veil of ignorance and enables us to realise the sublime vision of unity of Self. Some say that religion is a luxury. Those who say that religion is a luxury are sunk in ignorance and worldliness. How can a man live without religion. Religion only enables him to get rid of delusion, sorrow, pain and death and shows him the way to attain the summum bonum or the abode of immortality. Through religion alone he can become perfect and faultless.

That atrial or AV synchronous pacing may reduce the incidence of subsequent AF in patients with bradycardia compared with ventricular pacing.807, 808 On the other hand, studies in patients with intermittent atrial tachyarrhythmias failed to illustrate a general benefit of atrial pacing.808, 822, 897 Another potential avenue for primary prevention has been suggested following secondary analysis of placebo-controlled trials of treatment with ACE inhibitors.36, 749 In the LIFE41 and CHARM898 trials, the angiotensin receptor antagonists losartan and candesartan reduced the incidence of AF in hypertensive patients with LVH41 and symptomatic HF, 40, 898 respectively. These results, together with their favorable safety profile compared with antiarrhythmic agents, suggest a role for ACE inhibitors or angiotensin receptor antagonists for primary prevention of initial or recurrent episodes of AF associated with hypertension, MI, HF, or diabetes mellitus. An overview of 11 clinical trials involving more than 56 000 patients with different underlying cardiovascular diseases suggests that ACE inhibitors or angiotensin receptor blockers may reduce the occurrence and recurrence of AF.43 Yet inadequately explored, the use of statins has also been suggested to protect against AF, 335, 899 and dietary lipid components may influence the propensity of patients to develop AF.900 In 449 patients with CAD followed for 5 y, statin therapy reduced the incidence of AF--an effect not observed with other lipid-lowering drugs.899 In a canine sterile pericarditis model, atorvastatin prevented atrial electrophysiological and structural changes associated with inflammation and reduced the incidence of AF.119 Insufficient data are available at this time to permit recommendations for primary prevention of AF in populations at risk using dietary interventions, pharmacological interventions, or pacing or other devices.
